Rates
- $200 for an initial assessment
- $262.50 for 61-90 minutes
- $175 for 53-60 minutes
- $150 for 33-52 minutes
- $87.50 for 16-32 minutes
Payment
We accept cash, check and all major credit cards as forms of payment. HSA cards may be used as well.
No Surprises Act Notice
Questions to Ask your Insurance Carrier(s) Before Scheduling Therapy:
- Does your policy cover mental health services and therapy? *Do you have a Copay or Deductible plan for Therapy? If so, the amount? If Deductible, the coinsurance amount after deductible is met?
- Is there a limit on the number of sessions per year? If so, what is it?
- If seeking family therapy/Couples - Does the policy cover family therapy?
- If you are self pay due to having an out of network / non-credentialed policy - do they accept out of network claims from members, and if so what is the reimbursement rate you can expect to receive.
Insurances and Self Pay We have clinicians accepting one or more of the following:
Wellmark BCBS, HealthPartners UnityPoint, Self Pay, Medicaid, Iowa Total Care, WellPoint, and Molina. Please note, we are not credentialed/participating with Medicare.
We accept self-pay for most out-of-network / nonparticipating insurance providers. You would be self-pay for our clinic and then provided a receipt after full payment.
Depending on your policy you may or may not receive all or some of your benefit and it may process differently as out of network or even non-covered for being out of network.
Cancellation Policy
Appointments missed or cancelled within 1 hour of the appointment time have a $175 No Show fee.
Appointments Cancelled 1-24 hours in advance have an $87.50 Late Cancel Fee.
Please advise in case of emergencies and/or unavoidable events causing a Late Cancel or No Show.
Late Cancel and No Show fees are not covered by insurance and are client responsibility.
Insurance Filing Policy
Full Heart Family Therapy will submit your in-network claims on your behalf based on the insurance information provided. If your claim is denied or rejected we will work the claim and resubmit it. If we need follow up information from you we will reach out to you. If we do not hear back from yout or if the rejection or issue continues, the sessions will be changed to self pay, and follow the self-pay policy. We can then provide you with a receipt, often referred to as a Superbill, for you to submit at your discretion.
